Scientists zero in on origins of monkeypox outbreak

NEW YORK (NYTIMES) - When the first monkeypox cases were identified in early May, European health officials were stumped. The virus was not known to spread easily among people, let alone infect dozens - and soon hundreds - of young men.

The origins of the outbreak are now becoming clearer. Genetic analysis suggests that although the monkeypox virus is rapidly spreading in the open, it has been silently circulating in people for years.

Health officials have already identified two versions of monkeypox among American patients, suggesting at least two separate chains of transmission.

 

Researchers in several countries have found cases with no known source of infection, indicating undetected community spread. And one research team argued last month that monkeypox had already crossed a threshold into sustainable person-to-person transmission.

The genetic information available so far indicated that, at some point in the past few years, the virus became better at spreading between people, said Dr Trevor Bedford, an evolutionary biologist at the Fred Hutchinson Cancer Research Centre in Seattle.

"Genomic patterns would suggest this occurred around 2018," he said.

 
 

If the virus has adapted to include people as hosts, monkeypox outbreaks could become more frequent and more difficult to contain. That carries the risk that monkeypox could spill over from infected people into animals - most likely rodents - in countries outside Africa, which has struggled with that problem for decades. The virus may persist in infected animals, sporadically triggering new infections in people.

 

"We can also transmit this back to animals that can spread the disease within wildlife and back to humans," said Dr Sagan Friant, an anthropologist at Pennsylvania State University who has studied human-animal interactions in Nigeria for about 15 years.

The longer it takes to contain the virus, the higher the odds that it will find a permanent new home in people or animals, he said.

As of Wednesday (June 22), the United States had identified 156 cases in 23 states and the District of Columbia. The global toll has surpassed 3,400 confirmed cases, and another 3,500 cases are being evaluated, tripling the numbers from two weeks ago.

 

In Africa, eight countries had reported more than 1,500 suspected cases and 72 deaths as of June 10, most of which were in Congo.

Monkeypox is a large double-stranded DNA virus, about seven times as large as the coronavirus. DNA-based viruses can correct their own errors when they replicate their genetic material. They may collect just one or two mutations per year compared with 20 to 30 mutations for an RNA virus such as the coronavirus.

But the monkeypox virus seems to have amassed an unexpectedly high number of mutations - nearly 50 compared to a version that circulated in 2018, according to preliminary analyses.

Of the 47 mutations identified in one analysis, 42 carry the distinct signature of an enzyme called Apobec3. This enzyme, first discovered by researchers studying HIV, is a so-called host defence factor - an immune-system weapon that animals and people use to disarm viruses such as monkeypox.

 

The enzyme essentially forces viruses to make mistakes when they try to replicate, causing them to self-destruct. Mice carry just one version of this enzyme, while humans have seven. The rapid accumulation of mutations, characteristic of the enzyme since 2018, suggests that monkeypox may have switched to people as hosts around then, Bedford said.

It is unclear how the mutations might change the virus. Of 48 mutations identified in Britain, 21 may affect how the disease spreads, its severity and how well it responds to a treatment called tecovirimat, according to the United Kingdom Health Security Agency.

But because mutations introduced by the enzyme Apobec3 are meant to harm the virus, their quantity alone is not worrying, said Dr Michael Malim, a virus expert at King's College London who discovered Apobec3 in 2002.

The effect of the mutations is "more likely to be debilitating," he said. Comparing the current version of the virus with samples from the past few years might help understand how it has evolved, but that information is scarce. Nigeria did not have the ability to sequence genetic material until 2017.

Since then, scientists there have been analysing the sequences from about 50 monkeypox cases, according to Dr Ifedayo Adetifa, director of the Nigeria Centre for Disease Control. But without the specialised equipment or expertise needed for rapid analysis, the scientists have not yet completed their work, he said.

 

Although the researchers have fielded several requests for the data from outside Nigeria, Dr Adetifa said they would wait to publish their work to prevent teams with more resources from out-competing them and grabbing credit.

"I'm all for open data sharing and all of that," he said. "Question is, who benefits?"

Some experts have cautioned for years that the eradication of smallpox in 1980 left the world vulnerable to the broader family of poxviruses and raised the odds of monkeypox evolving into a successful human pathogen.

In West Africa, the incidence of monkeypox has increased at least twenty-fold since 1986. In African countries generally, Dr Adetifa said, "we suspect some under-reporting because there's been relatively low awareness and maybe low perceived risk of monkeypox."

Nigeria is stepping up its surveillance of monkeypox, and case numbers may rise as more people become aware of the virus, he added.

Although monkeypox has a distinctive rash that appears on the palms of the hands and soles of the feet, it is frequently confused with chickenpox. Many men in the current outbreak have lesions on their genitalia, but those can be mistaken for sexually transmitted infections such as syphilis, gonorrhoea and chlamydia.

Researchers in Italy and Germany have reported finding monkeypox DNA in semen, but it is unclear if the virus spreads that way or is merely present in semen and vaginal secretions.

The spread among young men with genital ulcers was observed at least once before. In 2017, Nigeria recorded 228 suspected cases of monkeypox and confirmed 60. The virus spread primarily among young men who had genital ulcers.

 

Britain's experience indicates how complicated it can be to trace contacts of a virus that may be sexually transmitted, especially in cases where infected people have had multiple anonymous partners. In an initial analysis of a subset of cases, officials said they were able to obtain names for fewer than one-third of the 78 reported sexual contacts.

Many cases in Africa have been traced back to contact with wild animals or the use of animal products for medicinal or cultural practices.

As deforestation and urbanisation drive people and animals into closer quarters, more viruses may make the jump to human hosts. Monkeypox is most likely to leap to people from rodents.

There are about 2,000 species of rodents worldwide, composing 40 per cent of all mammalian species. The African rope squirrel is a leading candidate as the primary reservoir for monkeypox, but there are other contenders, including striped mice and dormice, giant pouched rats, rusty-nosed rats and brush-tailed porcupines.

In a 2003 outbreak in the United States, a shipment of Gambian pouched rats imported from Africa transmitted monkeypox to prairie dogs, which then infected 71 Americans. But officials did not find signs of the virus in animals in the United States once the spate of cases had ended.

There's no guarantee that luck will hold this time. "These spillovers from other species, and what that means and what the trajectory is - it's very unpredictable," Dr Malim said. "And it's occurring more and more."

Most people will not go out to party if they have a fever.  A few days after the fever, the monkeypox started to appear which is visible to everyone. Then progressively matured, which is when it is most contagious. Contrary to convit virus and HIV, monkeypox can be seen,  when it matures, so to speak.  You can check for signs of the pox when people are naked, especially at massage parlors, swimming pool or suanas, and start chasing them out before thoroughly cleaning those areas.  We shouldn't be too concerned if we know there are not many cases in Singapore.  Unless you are the "lucky" one.

Study on monkeypox outbreak shows differing symptoms

A new study published in The Lancet Infectious Diseases found that the clinical profile in recent monkeypox cases is different than in past events, with fatigue and fever less common and genital skin lesions more likely.

 

This is the first study to look at these differences among cases in the current outbreak, and points to the important role sexual transmission among mostly men who have sex with men (MSM) has played in spreading monkeypox in non-endemic countries, many of which had never seen a monkeypox case before.

 

Most had 5 or more new partners

In the study of UK cases, all 54 of the participants identified as MSM, and their median age was 41. The patients presented to UK sexual health clinics during a 2-week period in May, at the beginning of the global outbreak.

 

None of the men had traveled to sub-Saharan Africa, but many reported recent European travel. Of the 54 men, 52 said they were not aware of being in recent contact with anyone with monkeypox. 

 

Forty-seven of 52 men who answered questions on recent sexual history said they had at least one new sexual partner in the 3 weeks prior to symptoms, and 29 of 52 (55.8%) had more than 5 new partners in the 12 weeks prior to diagnosis.

 

Almost all (49/52) patients presented with lesions and rashes on their genitals or anal regions. Only five patients required hospitalization, and no one died. One-quarter of the patients also were diagnosed with another sexually transmitted infection at the same time as monkeypox.

 

"The commonly observed symptom of skin lesions in the anal and penile areas, and the fact that a quarter of the patients tested positive for gonorrhoea or chlamydia at the same time as the monkeypox infection, suggests that transmission of the monkeypox virus in this cohort is occurring from close skin-to-skin, for example in the context of sexual activity," said Ruth Byrne, MD, from the Chelsea & Westminster Hospital NHS Foundation Trust, in a press release on the study.

 

Study authors also suggested that the primary symptom of genital rashes should be added to case definitions of monkeypox to more accurately identify symptomatic individuals.

 

There are now more than 5,700 cases of monkeypox in non-endemic countries, mostly in Europe and North America.

 

Iowa, Wisconsin, Connecticut report first cases

US cases of monkeypox are continuing to grow, with Iowa, Wisconsin, and Connecticut reporting their first cases of the virus. According to the Centers for Disease Control and Prevention, the country has 460 cases in 32 states.

 

In Iowa, health officials said the patient likely acquired the virus when traveling internationally. In Connecticut, health officials confirmed the patient was a man in his 40s from New Haven.

 

California has the most the most cases in the country, with 95, followed by 90 cases in New York. Illinois has 53 cases.

GENEVA (AFP) - The World Health Organisation (WHO) said on Tuesday (July 5) that 5,322 laboratory-confirmed cases of monkeypox had been reported to it in the current outbreak, 85 per cent of which are in Europe.

 

Though case numbers are rapidly increasing, the United Nations health agency has not set a date for its emergency committee on monkeypox to convene for a second meeting.

 

"From Jan 1 to June 30 this year, we have 5,322 laboratory-confirmed cases and one death," WHO spokeswoman Fadela Chaib told reporters in Geneva.

The number has increased by 56 per cent in eight days.

 

The previous figure given by the WHO, for the period up to June 22, was 3,413 cases.

A surge in monkeypox cases has been reported since early May outside the West and Central African countries where the disease has long been endemic.

 
 

Ms Chaib said infections had now been reported in 53 countries.

 

"Eighty-five per cent of cases are in Europe, followed by the African region, the Americas, the Eastern Mediterranean and the Pacific," she said.

"The WHO continues to ask countries to pay particular attention to monkeypox cases to try to stop further infections."

 

Most monkeypox infections so far have been observed in men who have sex with men, of young age and chiefly in urban areas, according to the WHO.

The normal initial symptoms of monkeypox include a high fever, swollen lymph nodes and a blistery chickenpox-like rash.

On June 23, the UN health agency convened an emergency committee of experts to decide if monkeypox constitutes a so-called Public Health Emergency of International Concern (PHEIC) - the highest alarm that the WHO can sound.

 

But a majority found the situation had not yet crossed that threshold.

 

However, they acknowledged the emergency nature of the outbreak and said controlling its further spread required intense response efforts.

The committee can reconvene at any time, depending on the changing situation.

 

The WHO's 16-member emergency committee on monkeypox is chaired by Dr Jean-Marie Okwo-Bele from the Democratic Republic of Congo, who is a former director of the WHO's Vaccines and Immunisation Department.

Absolutely,  it would be better if the case was linked to the imported one. If this guy got no travel history in the past weeks,  then yes, the virus is already circulating in the community. Everything rising, covid, TB, dengue and monkeypox next?

The problem is that monkeypox has a long incubation period of 21 days, which means 21 days you can be asymptomatic, not knowing you are infected, same as your sex partner.

 

The pictures can also be misleading. I feel pity for all guys suffering from strong acne...

 

But at least they help as a warning once monkeypox is visible...  My point was more that people with a fare skin would be less well knowing whether they caught monkeypox, when all the media publishes the symptoms on darker skin people...

Maybe the Government should offer anonymous vaccinations for Men who have sex with men (MSM), just like the anonymous testing...

 

In the UK and other European countries they started giving the vaccines to MSM.

Most assumably from a sex contact with someone who was overseas and returned to Singapore, unless the Malaysian guy living in Singapore travelled himself overseas.

The article does not say anything whether the guy was overseas in the last 21 days...

 

Usually when the local media writes "social contact" I know how to interpret it...

 

They just said that the other case of June is not linked but you can never say this with certainty because did the Brit disclose all his potential contacts and sex contacts?

The patient is a 42-year-old British man who works as a flight attendant and was in Singapore between last Wednesday and last Friday (June 15 and 17) and again on Sunday as he flew in and out of the country, said MOH on Tuesday night.

He tested positive for monkeypox on Monday and is currently warded at the National Centre for Infectious Diseases (NCID).

His condition is stable, said MOH, adding that contact tracing is ongoing.

Thirteen close contacts have been identified as at Tuesday.

SINGAPORE - The Ministry of Health (MOH) on Thursday (July 7) confirmed another imported case of monkeypox in Singapore.

 

The patient is a 36-year-old male Indian national who lives in Singapore and had recently returned from the United States, said MOH.

 

He tested positive for monkeypox on Thursday and is currently warded at the National Centre for Infectious Diseases (NCID).

 

The man is not linked to the earlier monkeypox cases, the ministry said. It added that contact tracing is ongoing.

 

"The case developed anal discomfort on June 28, and other symptoms, including rashes typical of monkeypox, progressively over the next few days," MOH said.

On Wednesday, the man sought medical care and was taken to NCID on the same day.

 
 

Last month, Singapore reported its first imported monkeypox case since 2019 - a 42-year-old British man who works as a flight attendant.

 

The country reported its first local case of monkeypox on Wednesday - a 45-year-old male Malaysian national.

 

He tested positive on Wednesday, and is currently hospitalised at NCID. His condition is stable.

 

All three cases are not linked.

 

MOH said that the risk of monkeypox transmission remains low given that it requires close physical or prolonged contact. 

 

Monkeypox is typically a self-limiting illness where patients recover within two to four weeks, said MOH, although a small percentage of those infected can fall seriously ill or die.

 

Those particularly vulnerable to complications are young children, pregnant women or those who are immunocompromised. 

 

MOH added that the monkeypox situation would continue to be monitored closely and public health responses would be calibrated as necessary. 

 

Returning travellers, especially those from areas affected by monkeypox, should seek immediate medical attention if they develop any disease symptoms within three weeks of their return, and inform their doctor of their recent travel history.

Confused,  so did he went back malaysia and make 14 casual contacts (then his case is not local liao)  or 14 casual contacts came over to SG?

 

Anyway this drama keep rolling into something big, waiting for them to annouce the social contacts for the 3rd and 4th cases. Pity those nurses,  just tot can take a rest from covid but cannot.

The Star is a Malaysian news agency. When they say "in the country", it means in Malaysia. So when "he went back malaysia and make 14 casual contacts", that means the 14 casual contacts are still in Malaysia.

Thanks,  got it.... So in this case there is a chance for him to kena in penang, then SG are still safe for now if all are imported cases. 

 

Friends and relatives just say so,  "casual contacts" or "social contacts" quite misleading ha.... 

 

If SG have,  i m sure Malaysia and Thailand will have too,  just a matter of time.

Back to the topic,  i dont believe other SEA countries dont have,  is just like covid, first reported was SG,  then Malaysia,  Thailand and finally Indonesia. It would be tough for those in the religious countries to explain how they got infected since monkeypox is associated mostly with MSM.
