Using condoms consistently for anal sex is the best solution. From what I know, you need to monitor your blood and kidney functions while on PrEP. If you bareback while on it, you can catch other STDs. I have been sexually active for the past two decades, and have never contracted STDs because I stick to the condom use. Nowadays, HIV infection is no longer a death sentence. But still why do we want to deal with a chronical disease which can be prevented?

PrEP doesn't protect you from all the other STIs out there....and even for HIV it's only 90-something % effective, so there's still a risk of getting HIV.

 

In an ideal world where everyone gets tested regularly, HIV negative people all take PrEP, and HIV positive people take their meds religiously, both/all parties do their part to prevent the transmission of HIV and the risk will truly be negligible. Unfortunately , I think we've still got some ways to go before we can reach that. Until then please remember that although PrEP does drastically reduce the chances of getting HIV, the risk still remains albeit small.

 

Do consider twice if you wish to undertake this risk that will affect the rest of your life. Even if you do get HIV meds you will still have to be on meds permanently. And your medical status may have effects on work, immigration, and other areas of your life. You may enjoy the bareback sex for say 30 mins or an hour, but the burden and other effects of HIV will haunt you for life.

Really tempted to delete it as almost 80% of them have status on prep, seems like many are having bareback sex. Maybe even more as not everyone are that honest.

On prep doesn’t mean they do bb. It’s just an added measures to deter hiv infection. Don’t assume. If Not sure, you can always say “strictly safe fun only”. If not sure about their status, ask. If not sure if you can trust them, then don’t do it at all.

Prep is a preventive measure. Like vaccines and other medication and supplements.

 You take flu jab is to increase your immunity to prevent getting flu. Does it mean you’re actively seeking to get flu from others?  You take vit c and other supplements/health booster cos you want to stay healthy and hope to prevent getting sick, that doesn’t mean you will or want to keep exposing yourself to sick people. 

 

I know people who take prep to reduce the chance of getting infected, and one can get infected by many ways other than bareback fuck. They just took it because they engaged in casual sex other than their partners and they wouldn’t want to infect their partners. That’s called being responsible (and pls don’t argue that being responsible is to not fuck around when you have bf, because fuck you you little holy innocent angel who think you’ll not fuck around when attached, not everyone is a cute little angel that you are)

How you best take PrEP depends on how far in advance you plan having sex, how regularly you have sex and not necessarily how much sex you have.
 
There are slightly different recommendations for anal sex and vaginal sex. Switching between dosing options depending on your sexual activity and patterns is totally OK. You can adjust your PrEP regime to best suit you as your situation changes.
 
It is important to be aware that EBD or on demand and 4 pills per week are only suitable for anal sex. Daily PrEP and Holiday PrEP are the only methods of taking PrEP that are suitable for both anal and vaginal sex.

The 
iPrEx study that led to the approval of PrEP in the US used daily dosing (1 PrEP pill per day) and the UK's PROUD studyalso used PrEP once a day.
 
The French and Canadian 
Ipergay study looked at taking PrEP on demand ie leading up to a specific sexual event rather than everyday to see if PrEP was also effective in this setting: it was. This study was mainly in men who have sex (MSM) and a small number of trans women. We know that a minimum of 6 pills per week is required for adequate protection for vaginal sex.

Both Ipergay and PROUD returned the same effectiveness rate of 86%. It's worth understanding that "effectiveness" is how well a prevention tool works at reducing transmissions within a group of people. If an individual uses PrEP as instructed it offers almost 100% protection from HIV. No one in either study (Ipergay or PROUD) who took PrEP as directed seroconverted (became HIV positive).

How to take PrEP

The 4 ways to take PrEP

Daily PrEP (1 pill per day)
The lead in time for daily PrEP is 7 days. It is suitable for both anal and vaginal sex.
If taking daily PrEP, it is best to take it at the same time each day so that it becomes part of a routine but a few hours late or early is fine.
 
You can keep your PrEP next to your tooth brush and tooth paste and take your pill each morning after you brush your teeth. It's little things like this that will help you remember to take your PrEP. PrEP is an 'opt in' treatment - it doesn't have to be forever.
 
Be kind to yourself and try to remember to take your pill every day. Your peace of mind and relieving the stress and anxiety that is sometimes involved in staying HIV negative is also hugely important. It’s not just about sex, it's about your head and your heart too.
You can take PrEP with or without food, and at any time of the day.
 
One of the benefits of taking daily PrEP is that you can miss a pill from time to time and still have adequate protection.
Event Based Dosing or on demand PrEP
 

This dosing option is only suitable for anal sex, not vaginal. If you are using PrEP on demand it is really important not to miss any doses.

Because Ipergay is the only study that looked at using PrEP in this way there has been a certain amount of hesitation from advocates and organisations but we are confident there is enough evidence and data from a real world setting to say that Event Based Dosing or on demand PrEP works for anal sex.

If you know that you might have condomless sex 24 hours in advance:

  • Take 2 pills 2 - 24 hours before sex

  • Take 1 pill 24 hours later

  • Take 1 more pill 24 hours after that

If you are having sex for an extended period of time perhaps over a few days or a weekend, continue to take a pill every 24 hours until you have 2 sex free days.

This option is not recommended if you have an active hepatitis B infection as the drugs in PrEP also supress the hepatitis B virus and as such starting and stopping PrEP can lead to viral flare ups and subsequent liver inflammation.

4 pills per week or sometimes called "the Ts and Ss"
 
This dosing option is only suitable for anal sex, not vaginal.
 
We recommend daily dosing for 7 days then stepping down to 4 pills per week.

4 pills per week usually involves taking a pill on Tuesday, Thursday, Saturday and Sunday hence "the Ts and Ss"

15 pills per month was the median (average) number of pills that people in the Ipergay study took, this equates to 4 pills per week. Data suggests that 4 pills per week offers almost the same level of protection as daily.

This option has several benefits. Some people who might only have sex once or twice per month might not want to take a pill everyday if it's only for protection for these 1 or 2 events. 4 pills per week will maintain a good baseline of drug in the system and some people choose to move up to 7 pills per week when they know they are in a more sexually active period.

Some people using PrEP on demand find that they might be taking 4 pills per week most weeks of the month and so opt for structuring this into the Ts and Ss instead.

This dosing option has the benefit of reducing the number of pills taken per month by almost half which in turn reduces the cost per month by almost half.

Although these dosing options don't appear to follow one simple rule of thumb, they are based on study data and as such study data doesn't focus on or contain much in the way of the minutiae of details of real life use of PrEP.

 

 

Holiday PrEP

If you are thinking of using PrEP for a pre-planned block of time where you anticipate your risk of exposure to HIV to be higher due to increased number of partners of unknown HIV status, situations where condoms are not easily or always used, where substances might be used or will be having sex while travelling to a country with a high HIV prevalence then this option is worth considering.
 
This option is suitable for both anal and vaginal sex.

Based on a 7 day period (or holiday) we recommend 7-7-7:

7 days daily dosing before the period
7 days daily dosing during the period (or for as long as the specific period lasts)
7 days daily dosing after the period.

There are a few reasons why we recommend 7 days pre and post. 7 days lead-in has now been recommended as providing adequate levels for both anal and vaginal sex. If you are planning PrEP around a holiday or working trip away and you experience any mild side effects (most people do not experience any side effects) then most of these should have subsided within a week, in time for the pre-planned block of time.
 
1 pot of PrEP (30 pills) will cover a 2 week block. People seem to be more comfortable using this method in week blocks although EBD/on demand as per Ipergay instructions states to dose only for 2 sex free days after the event.

Yep! Very good info to know =) If I know I'm going to have a good time with my regular after we have all done our checks (with 4th gen test kits), prep on demand isn't a bad option.

Yup, this is what I have been doing.

 

Since my sexual habits/frequency is quite low, maybe one hookup every 2 months.

It's better to not overload my liver and kidney with taking daily Prep.
if I do find myself having sex with more frequency, only then continue on with 1 per day.

I do feel a abit woozy taking two Prep at one go after a long period.

but the last thing i want is for my body to build a resistance,

which some sites are already reporting in some cases.
so i'd still prefer this than taking 1 daily.

 

also 4 tabs every few months, is also not that big of an expense,

making this preventive medication a feasible and cost-effective option

on top of condoms and other safety measures
