Believe you are inquiring about how to be exempted from the pre-employment medical examination. I would suggest you discuss this matters with the hiring manager. 

 

Companies are responsible to ensure that their employees undergo pre-employment medical examinations by a designated workplace doctor and be certified fit to work. These medical examinations should be done before or within three months of commencement of work. It is not discriminatory if candidates fail the test and are certified to be unfit as a result and are not offered the job in this scenario. If an offer has been made before the pre-placement test, companies should consider finding an alternative position that is suitable for the candidate.

Take unpaid leave or postpone the medical till after 3 month period. If mental or sensitive issues  take leave. By law, medical benefits including mc to be paid by company after 90 days. Same for paid leave. Some companies may allow earlier,if they cover by panel doc. Some smaller companies dont and have immediate managers approve all medical claims and leaves. The later is one to be wary of as some of people in supervisory positions use this as leverage to control you.

To maintain the privacy of your medical appointment, you can explore the option of taking parental care leave if applicable or urgent half day leave. If you're comfortable sharing with your supervisor, you can simply inform them about the medical appointment. Company policies vary, and it may result in time off or the use of annual leave. Alternatively, you can request a medical certificate to excuse your absence during that period.

Look at your employment contract. Usually it will say along the lines of, during probation, you will be allowed to take MC/leave that will be deducted from your annual if you pass the probation period, failing which, it will be deducted as no-pay leave.

 

It depends on your work nature and work culture if you want to disclose why/where you're going for your medical appointment and the duration. It could mean disappearing for the next two hours and then returning to your workplace and stay two hours later to show your diligence. If it's group work based, then arranging the medical based on less disruptive timing/days to your team mates will help too.

 

As for asking, just ask your supervisor and see if need to apply on the system or not and follow up with an email for documentation.

Unless you do not wish your supervisor/manager to know about your medical condition, just be upfront and let them know you would like some time off from work to attend your scheduled medical appointment.  Most bosses will understand.  If not, apply for time-off (against your annual leave entitlement).  Alternatively, request your Doctor to issue you a certificate of attendance for the attendance to support your requested time-off.  I am inclined to be always upfront with your bosses 😏.
